    Concordance between HB&L system and conventional culture method for bacterial antibiotic susceptibilities from positive blood samples at Hospital for Tropical Diseases from 03/2020 to 04/2021

    Presently, shortening the time-to-result of antimicrobial susceptibility test (AST) is of the utmost importance for sepsis management in an effort to reduce mortality and antibiotic resistance rate. HB&L ((Human Biological Liquid Culture), a product of the Alifax company (Italy), can detect replicating microorganisms and their antibiotic resistance in a few hours with high sensitivity and specificity by using light scattering technique. This study aims to evaluate the correlation between rapid antibiogram using the HB&L system and the routine methods on positive blood cultures at the Hospital for Tropical Diseases in Ho Chi Minh City from March 2020 to April 2021. Gram-positive bacteria blood samples were tested against Cefoxitin, Vancomycin while Gram-negative bacteria samples were trialed with Ceftriaxone, Levofloxacin, Piperacillin-Tazobactam and Meropenem. The concordance of the two methods was defined by the categorical agreement (CA), very major errors (VME), major errors (ME) and minor errors (mE) in Cumitech 31A. Of the 81 blood culture samples containing Gram-negative bacteria, the CA of Ceftriaxone, Meropenem, Levofloxacin and Piperacillin-Tazobactam were 90%, 100%, 67% and 88%. The maximum VME was 22% for Levofloxacin. On the other hand, of the 32 Gram-positive samples, CA and VME of Cefoxitin were 53% and 65% respectively while CA of Vancomycin was 100%. The median time to receive rapid AST results of the HB&L system is 4.43 hours faster than the standard that.

    In vivo evaluation of floating tablet clarithromycin 500mg

    Floating drug delivery system (FDDS) containing Clarithromycin 500mg (CLA) designed to prolong the retention time in the stomach is also an approach to optimize the plasma CLA concentration in the gastric. However, the development of FDDS meets many obstacles about physiology such as cannot control and locate FDDS in gastrointestinal because of the process of stomach emptying. CLA is rapidly absorbed from the gastrointestinal tract, bioavailability in intact form is only about 55% with a relatively short half-life, about 5-7 hours when taking a 500mg dose. Therefore, this research approaching a dosage form CLA to prolong the gastric retention time by in vivo in living body is local dog because the test subject is a dog beagle because anatomically it is quite similar in size to the gastrointestinal tract with the human model. The technical to show CLA image in vivo by X - ray method each time before and after oral administration of a pill and finished until there is no more pill images in the stomach. In addition, plasma drug concentrations were also calculated at each time point using the HPLC with LCMS IT-TOF method and compared with tablets without buoyancy properties. Results of research FDDS containing CLA has been the floating tablet time is 3,5 to 4,5 hours. And plasma concentration between testing group reached at 6 and 8 hours (1964,00 ± 104,25 ng/ml and 1203,00 ± 191,68 ng/ml); and the drud’s peak group reached at 2,5 -3 hours (1169,88 ± 109,42 ng/ml and 1982,63 ± 266,48 ng/ml), respectivety.

    Optimisation of a floating tablet containing clarithromycin 500 mg

    The purpose of the research is to optimize the formulation of a gastric floating drug delivery system of clarithromycin 500 mg (CLA). CLA is a macrolide antibiotic, indicated in the treatment of sinusitis, pharyngitis, pneumonia, acute and chronic bronchitis, especially in the treatment of Helicobacter pylori (H. pylori) infection. The formulation design is retained in the stomach for prolonging the floating time to improve bioavailability, drug absorption. The excipients include polymers such as hydroxypropyl methyl cellulose (HPMC) to control drug release. Besides, sodium bicarbonate is generally used in pharmaceuticals as a source of carbon dioxide in effervescent tablets – has been used as a gas-forming agent in floating to prolong floating time and reducing gastric irritation. Preformulating testing is not the interaction between the ingredients together. In vitro floating time and vitro dissolution studies were carried out that the results of the best formulation with HPMC K100M, HPMC K15M and NaHCO3 are 22%; 12.5% và 14.7% based on concentrate of CLA. The formulations were designed by Design - Expert and BCPharsolf OTP. The result of research showed that the floating time is approximately 8.53 – 9.34 hours and drug release for 12 hours, respectively 25% (after 2 hours), 20 – 40% (after 4 hours) and over 80% (12 hours). The analytical method for determination and quantification of CLA such as high-performance liquid chromatography (HPLC) and Fourier transform infrared FTIR.

    Factors influencing students’ satisfaction towards online learning platforms at the VNU-HCM School of Medicine

    This study investigates the factors affecting students’ satisfaction towards the online learning platform at the School of Medicine, VNU-HCM, by conducting a survey with 268 undergraduate students who have utilized Quickom and Microsoft Teams. The study employed SPSS 20.0 software to assess the reliability of using Cronbach's Alpha, the exploratory factor of EFA, the correlation, the multiple linear regression and ANOVA. The results from Quickom analysis showed that all of five independent factors in the model had a statistically significant impact on students’ satisfaction from high to low, including the perception of the usefulness (beta = 0.331), the convenience (beta = 0.279), the platform quality (beta = 0.242), the ease of use (beta = 0.118), and the technical barriers (beta = -0.114). The results from ANOVA analysis showed that there was no significant difference in the evaluation criteria between Microsoft Teams and Quickom. This study is hoped to serve as a Vietnamese additional source related to students’ satisfaction about online learning flatform and the field of medical student online training. Therefore, it helps policy makers and administrators to compile development strategies more concisely and effectively in accordance with the future trend of technology development.

    Massive infectious ovarian cyst represented as a massive ascites: A case report at Binh Dan hospital

    Cystic abdominal tumors are extremely common and now they are diagnosed more frequently and earlier due to availability of better imaging modalities. Giant ovarian cysts (GOC) are fluid-filled sac or pocket measuring more than 10 cm developed at the expense of the female gonad. GOCs are rare nowadays due to early diagnosis and appropriate management thanks to new diagnostic technology. Occasionally, infectious GOCs still occur. This case study describes how a substantial ovarian cyst might cause ascites in a postmenopausal lady to be misdiagnosed. Factors related to the late hospitalization of giant ovarian cyst have also been noted.

    Formulation of virgin coconut oil microemulsion with natural excipients

    Virgin coconut (Cocos nucifera (L.)) oil (VCO) is taking attention from re-searchers because of its biological activities. However, applying VCO directly on the skin often leaves an uncomfortable feeling of greasy, clogged pores and difficult to wash. Coco-nut oil in microemulsion is a better option since it can increase bioavailability as well as convenience for users. Furthermore, this study aims to use natural ingredients, safe for the skin and environmentally friendly.

    COVID-19 has become a pandemic and is still breaking out in many countries all over the world due to the emergence of new variants. Several safe and effective vaccines have been approved and succeeded in reducing the rate of patients with severe symptoms and that of mortality. Currently, most countries have chosen to change their policy from “zero Covid” to “live with” the pandemic and opened their boundaries to recover their economy. However, the emergence of new dangerous variants such as Delta or Omicron significantly reduces the effectiveness of vaccines and therefore, is complicating the pandemic control. In addition to the use of vaccines, the development of antiviral drugs is extremely important to tackle this worldwide threat. Currently, the approved anti-COVID-19 agents consist of two main categories, including spike protein (S) targeted monoclonal antibodies and orally available small molecule antiviral agents. However, the biopharmaceutical approach still incurs significant drawbacks such as bad bioavailability, expensive and growing resistance of the virus due to the accumulation of mutations in the receptor binding domain (RBD) of the spike protein in new variants. On the other hand, small molecule antiviral agents are a more appealing approach as their main therapeutic targets are intrinsic enzymes which are less likely to mutate, hence remaining inhibitory activity against different variants. Additionally, the oral availability of these drugs also allows these medications to be taken by individuals without the aid of healthcare professionals, which could alleviate the burden on healthcare systems. Therefore, the research and finding of new orally available antiviral agents is still a necessity. This review is carried out to give an overview of the discovery and development, mechanism of action, clinical trials, and approval status of current small-molecule anti-SARS-CoV-2 drugs.

    Psoriasis is a chronic inflammatory disease, not just affecting skin and joints but also associated with the development of cardiovascular diseases, metabolic sydrome. Secukinumab is demonstrated highly effective in the treatment of psoriasis, but so far, there has been few research on the effect of secukinumab treatment on hematological and blood biochemistry parameters in psoriasis patients. The correlation between the change of these parameters and the change of clinical outcomes are aslo evaluated.

    Long-term mask use can lead to skin reactions, such as itching, allergic dermatitis, irritant contact dermatistis, increased sebum secretion, acne formation or increased existing acne. Currently, there are many researches on the affection on skin of persional protective equipment for Covid-19 in general and face masks in detail. To our best knowledge, there has been no studies about this issue in Vietnam up to now.
